Sports Physicals for students 7 -12 grade will be done on Saturday, May 30th, 2009 from 8:00am until 11:00am @ SMFHS in the student Commons. Cost is $20.00.

Be sure to get the 4 page physical form at Kimpton Middle School, Stow-Munroe Falls High School or download here. Completely fill out the form (ALL 4 pages) and bring with you to your
scheduled time.
